Organizations respond with well-intended but ineffective corrective actions because they failed to address the root causes driving those incidents in the first place. In this session, safety leader Joe Clady shares a practical, culture-driven approach to root cause analysis and corrective action planning.
This session introduces a holistic approach to incident investigation and corrective action planning. Participants will explore the differences between root cause analysis (RCA) and corrective actions, with a focus on the most common breakdowns that lead to recurring failures.
Using a real-world case study of a fatal caught-in incident, attendees will explore how to:
Control the scene and gather accurate data
Apply situational, psychological, behavioral, and cultural lenses
Avoid investigator bias and shallow conclusions (like “human error”)
Identify systemic gaps rather than individual fault
Translate findings into specific, trackable corrective actions
The session culminates with a practical framework for crafting effective corrective actions, including assignment of responsibility, timelines, communication strategies, and integration with leading indicators and performance metrics.
